This PR includes race-condition fixes from ipfs/boxo#994 and increased DefaultMaxConcurrentRequests = 4096
If CI is green I'm merging it to fix master (which includes race condition introduced in #285 which used commit without fix) (cc @hsanjuan for visibility)
