This adjusts the miner to count the number of staking intervals that saw one or more successful UTXO mining loops. A measure of staking loop efficiency is then calculated by dividing that counter by the total number of intervals that have elapse since wallet start up. The is a good measure of staking coverage.

This is a bug. There is no reason that the miner should return from a successful pass in the target window without updating the other g_miner_status fields. I implemented a boolean flag and changed to a break, and then unified the handling below.

This modifies the original implementation of the MilliTimer class in the miner in #1983 and makes some improvements, including being able to specify by timer whether the timings should be output to the debug log.
It also uses the improved class to clean up the timing code in the miner and init areas, implement uptime report in getinfo, and implement a stake loop efficiency measure in getmininginfo.
