What happened?

I was in plan mode. I went to act. The implementation went rogue, adding interfaces that existed. I canceled, then restored task & files, switched back to PLAN mode, and added comments to focus the implementation. I submitted my comments and it proceeded as though it were in ACT mode to make changes to my code.

Steps to reproduce

1. Enter PLAN mode
2. Submit task prompt
3. Switch to ACT mode
4. Cancel task
5. Restore task & files
6. Switch back to PLAN mode
7. Submit additional task text
