What happened?

I've observed this started happening close to the release of openAI's o3mini, but this might be just a coincidence.

The crux of the matter is that while PLAN makes changes and does not even show the Changes.

In PLAN mode should never write files or make changes.

This is happens mostly with o3-mini (which i use mostly now)

Steps to reproduce

Provide some file references to a task via PLAN.
in many cases it will just do the changes and will not show the new changes.

After you press ACT it will show the changes made before (but not do anything)
since it was already done via PLAN.
