Issue body actions

1. Collapse the working set by default. Show diffs.
2. Inline summaries are disabled by default and the setting is hidden for now while we re-work it into checkpoints. (@aiday-mar work). This is configurable with chat.checkpoints.showFileChanges
3. Remove items from working set once they are "Kept"
