FYI @thomhurst, we think that
ITestHostApplicationLifetimeis a better name as it makes it more clear that it's a "test host" extension point and that it doesn't apply to test host controllers. In fact, we may consider introducing a similar extension point for test host controller (and orchestrator as well if/when we expose it)
