Improve GitHub issue templates:

Remove default text in GitHub issue template titles.

Text like the following is redundant, because we already have labels (like 🐛 bug) to denote this info.

🐛 [BUG] 

If a label ever gets switched, then we must also update the title. I've seen inconsistent titles & labels.

Other cleanup.
